With regard to the two points you've raised, TGM teaches that, normally, the Body is 'comparatively' squared away at Impact (10-8-A). For special situations (especially Short Shots) the entire Body (not just the Stance) can be well Open to the Target Line (10-8-B).

The Right Foot is normally Flat well into the Downstroke and perhaps even into the Follow-Through. When the Heel does come up, it should be because the Pivot pulls it up, not because it is artificially lifted up. As with many other virtues, it is possible to exaggerate the Flat Right Foot into a fault. As a general rule, though, I see more artificial lifting of the Heels -- Happy Feet! -- than necessary.

Also, extraneous Knee Motion can be a problem. Remember, your Knees are your Anchors. Their function is to keep the Head Stationary, and that becomes difficult to do if the Knees are out of control and firing all over the place.
