Do you video?

Could be that you are over doing the trigger delay deal. If you keep your left hand turned to the plane too long you could actually be "stuck" and leading with the hosel. You can shank with too much "out" and with too much "in."

What are you TRYING to do with your swing?

What are your bad shots doing? Trajectory? Start line? Curvature?
