When you returned your Hands to Adjusted Address, you allowed (inadvertantly) your Right Elbow to Move to the Right and away from your body (off-Plane) which Rolled your Right Hand (and Left) and Closed the Clubface. Move your Right Elbow a little Left (toward your body) until your Clubface once again appears square to target. Don't tell anyone your Right Elbow was out of alignment.
