This is a guess. At impact your hands are not leading enough and there is NO forward lean to the shaft. Let me give a image to try. Imagine the club is a loaded rifle and you grip the barrel. At address the rifle points at you but as you take it back and cock it then the barrel does not point at you until after follow through. At impact the gun goes off but is aimed well outside your left hip. A scary image but makes the point. Don't shoot yourself in the hip, or worse, hit another bad shot.
